Social Worker


Social Workers interact with the patient, family and rehabilitation team (including insurers) and can assist in the following ways:

Evaluate the patient’s total living situation, including lifestyle, family finances, employment history, and community resources, and assess the impact of disability or disease on these areas.

Maintain a continuing relationship with the patient and family.

Coordinate funding resources and discuss financial arrangements and concerns. This may include a case management role.

Help the family develop the skills needed to participate actively in treatment procedures in the home.

Provide assistance in locating alternative living situations.

Assess vocational barriers.

Provide emotional support to patient and family in stressful situations.

Facilitate discharge planning.
